Case overview and current status
The Thirlwall Inquiry published its report on 15 September 2026 concerning events at the Countess of Chester Hospital from 2015 to 2018. The inquiry examined governance, patient safety and safeguarding following Lucy Letby’s criminal convictions. It was a public inquiry, not a criminal appeal. Letby’s convictions and any Criminal Cases Review Commission process must be reported separately from the inquiry’s findings.

Evidence and separate legal processes
Public inquiry: examines systems and institutional decisions. Operation Duet: ongoing police investigation into potential corporate and individual criminal liability. Criminal convictions / CCRC: separate criminal judgments and any statutory conviction-review process. Neither inquiry findings nor police investigations automatically alter a conviction.
Unresolved questions
Which recommendations will be implemented, what decisions will police and prosecutors make concerning Operation Duet, and what is the status of any CCRC review? Verify each against the responsible agency before publishing further claims.
